When Care Needs Become Clinical
Complex care is support for people whose needs go beyond what a standard care agency is set up to deliver. It usually involves a long-term condition, a disability or a clinical requirement that has to be managed daily and managed correctly.
Families often arrive here after being turned away. A discharge is delayed because no provider will take the package. An agency withdraws when needs escalate. A hospital says the only option is a nursing home.
That is where being nurse-led stops being a description and starts being the difference. Our clinical team assesses, plans, trains and supervises the care directly, which means we can take on packages that non-clinical agencies cannot safely accept.

What Nurse-Led Means in Practice
A registered nurse assesses before care begins
Not a coordinator with a checklist. Clinical risk, skin integrity, medication, nutrition and moving and handling are all assessed properly at the outset.
Carers are trained for this person specifically
Competency-based training on the individual's own equipment and routine, signed off by a nurse before a carer works unsupervised.
Clinical oversight continues
Care plans are reviewed as a working clinical document. Carers know what deterioration looks like, and concerns are escalated to our nursing team the same day.
We work alongside the wider team
District nurses, GPs, tissue viability, community matrons, therapists and hospital teams. Complex care fails when providers work in isolation.
How We Deliver Complex Care
Complex packages are built around a small, consistent team rather than a rota of whoever is free. Continuity matters more at this level than at any other, because a carer who knows someone well notices the change that a stranger would miss.
Every carer on a complex package is fully referenced, DBS checked and trained to the Care Certificate, with additional condition-specific training completed and signed off before they work alone. Where two carers are required for safe moving and handling, both are trained on that person's own equipment.
Packages can be delivered as visiting care, live-in care or full 24 hour cover with waking nights, depending on what the assessment identifies. Behind all of it sits our nursing team and an office staffed by an experienced member of staff 24 hours a day.

Information for ICB, NHS and Social Care Referrers
We accept referrals from ICB continuing healthcare teams, hospital discharge coordinators, community matrons, social workers, GPs and case managers. We are equipped for clinical packages, including those other providers have declined, and for rapid discharge where a delay would keep someone in a bed they no longer need.

Nurse-led assessment
A clinical assessment in the person's own home, covering health, risk, equipment and moving and handling.
Training and sign-off
We recruit or assign the team, complete condition-specific training and sign off competence before care begins.
Care begins, and is reviewed
The plan is a live clinical document, reviewed as the condition changes and adjusted with the wider healthcare team.
Paying for Complex Care
NHS Continuing Healthcare
Where needs are primarily health related, NHS Continuing Healthcare may fund the full package. Complex care is the service where CHC most often applies, and we can help you understand whether the criteria are likely to be met.
NHS Funded Nursing Care and Section 117
Both can contribute to clinical care at home where the criteria are met.
Local authority, direct payments and self-funding
Hampshire County Council may contribute after assessment. Direct payments let you choose your own provider. Self-funders receive a written rate sheet before care begins, with at least thirty days notice of any change.
Frequently Asked Questions
Another agency has said the needs are too complex. Will you look at it?
Yes. Tell us what has been declined and why. We cannot promise to take every package, but we will assess it properly rather than refusing on the phone.
Are your carers nurses?
Our carers are trained care staff working under registered nurse assessment, training and supervision. Where a package requires registered nurse input directly, we will tell you and arrange it accordingly.
Can complex care be delivered overnight?
Yes. It can be visiting care, live-in care or full 24 hour cover with a waking night carer, depending on what the assessment identifies.
Will you work with our district nurses and consultants?
Yes, and we expect to. Complex care only works when the provider is part of the wider team rather than operating separately.
How quickly can a package start?
The assessment can usually happen quickly. The package itself depends on training and sign-off, which we will not shortcut. We will give you a realistic date rather than an optimistic one.
What happens if needs escalate?
We reassess rather than withdraw. Escalating need is exactly what a nurse-led service is built to absorb.
