What Happens After You Send This
We call you, within 24 hours
Sooner if your situation is urgent. It is a conversation, not a sales call.
We arrange a visit at a time that suits you
Usually within a few days, and quicker where a hospital discharge or an emergency is driving the timing.
The assessment itself
Our clinical team comes to the home, meets the person needing care and talks through what would genuinely help.
A written plan and costs
You receive the proposed care plan and a written rate sheet. You decide in your own time, and you are free to say no.
What We Look At
- Health needs, medication and any clinical requirements
- Mobility, moving and handling, and the risk of falls
- Personal care, and what help is wanted rather than assumed
- The daily routine, meals, sleep and what a good day looks like
- The home itself, including any equipment or adaptations needed
- What family and friends are already doing, and where the pressure is
- Preferences about carers, including whether a male or female carer is preferred
- Funding, and which routes may apply
You do not need to prepare anything. If you have a hospital discharge letter, a list of medication or a recent care assessment, it helps, but nothing is required.

Things Worth Knowing Before You Send This
It costs nothing and commits you to nothing
The assessment is free whether or not you go on to arrange care with us. Nobody will pressure you, and we will tell you honestly if we think something else would serve you better.
You are not signing up to anything long term
There is no long-term contract. Care can be increased, reduced or stopped as circumstances change, subject only to the notice in the care agreement.
Your information stays private
What you tell us is treated as confidential health information, stored securely and shared only with your consent or where we are legally required to.
If You Need Someone Sooner
If a discharge is imminent, a family carer has been taken ill or something has happened today, please call rather than filling in the form. Our office is staffed by an experienced member of the team 24 hours a day, and we will tell you immediately what we can do.
