Coming Home Should Not Be the Hard Part
Discharges rarely happen on the timescale families expect. A ward decides someone is medically fit, and suddenly there are two days to arrange care that nobody has thought about yet.
We are used to this. Our office is staffed by an experienced member of the team 24 hours a day, and a significant part of our work over the last decade has been packages arranged at short notice, including late in the day and over weekends.

What We Can Arrange
Care from the day of discharge
Visits timed around the return home, so someone is there when the transport leaves rather than the following morning.
Round the clock cover for the first stretch
Live-in care, night sitting or 24 hour cover with a waking carer for the first days or weeks, stepping down as confidence returns.
Clinical needs after discharge
Catheter and stoma care, complex medication, skin integrity and repositioning, all under nurse-led oversight.
Getting the house ready
Shopping, a meal in the fridge, heating on, prescriptions collected and the bed made up. Small things, but they are usually what nobody has had time to do.

The Weeks That Decide Whether It Works
Readmission usually happens for ordinary reasons rather than dramatic ones. Someone stops eating properly. A new medication is not taken as prescribed. A small fall goes unmentioned. Confidence drops and movement drops with it, and within a fortnight the person is weaker than the day they came home.
Our carers are in the house daily during that period, which means those things get noticed early. Appetite, hydration, medication, mobility, mood and skin are all monitored, and anything concerning goes to our nursing team the same day and on to the GP or district nurse where needed.
Support can then reduce as recovery allows. Many discharge packages start intensively and step down within weeks, and we would rather reduce your hours than keep billing for care that is no longer needed.

Information for Discharge Teams and Referrers
We accept referrals from hospital discharge coordinators, discharge to assess teams, ICB continuing healthcare teams, community matrons, social workers and brokerage, including packages other providers have declined.
Funding routes we work with: discharge to assess, CHC Fast Track, NHS Continuing Healthcare, Funded Nursing Care, Section 117, local authority and direct payments.

Who Pays for Care After a Discharge
Short-term NHS funded care
Many people leave hospital with a period of NHS funded reablement or discharge to assess support before any charging decision is made. Ask the ward or the discharge coordinator what has been arranged, because families are frequently not told.
CHC Fast Track
Where someone is leaving hospital with a rapidly deteriorating condition, a clinician can complete a Fast Track application, which can fund the full package quickly.
Local authority, direct payments and self-funding
Hampshire County Council may contribute after assessment. Self-funders receive a written rate sheet before care begins, with no long-term commitment, which matters when a package may only be needed for a few weeks.

Can care start the same day someone comes home?
Often yes, depending on the needs and the notice. We aim to have a carer present for the arrival home rather than the following morning.
Do we have to commit to ongoing care?
No. Discharge packages are frequently temporary. There is no long-term contract and support can be reduced or stopped as recovery allows.
Can you take clinical needs, not just personal care?
Yes. Catheter care, stoma care, complex medication and skin integrity are all within our nurse-led service.
What if the discharge date moves?
That is common and it is not a problem. Tell us and we will adjust the arrangements.
